NEW INTEGRATED DIABETES CARE SERVICE STARTS FROM 1st APRIL 2010
The Integrated Community Diabetes Service, (ICDS) provided by UHL is due to start in April 2010. Care will be provided closer to home for patients who have poor control of their diabetes through eight community based clinics. This will mean patients who previously attended hospital as an outpatient will instead, where appropriate, be able to receive specialist care at health centre’s and medical practices. Our vision is to provide and integrated model of care responsive to the needs of our local community
The Way Diabetes Services Are Delivered For Some People With Diabetes Is Changing?
From April 1st 2010 if you live within NHS Leicester City area you may be able to access your diabetes care closer to home and a choice of location that suits you.
This innovative new service will be available to those people with diabetes requiring additional support and help with their diabetes management.
WHO WILL BE DELIVERING THE SERVICE?
The team delivering this service will have the right skills to support people with diabetes requiring more complex care within their own communities. (UHL Diabetes Specialist Nurses (DSN's)) For some people the change will be more immediate as they will be able to be transferred directly into the new service.

Why Is The Service Changing?
The developments are taking place to ensure we continue to provide good services that help people with diabetes live healthier lives.
How Is This Change Better For Me?
Clinic locations will be closer to home and easily accessible.
Does This Mean I May Have To Change My Gp For All My Care?
No. Your existing GP will continue to look after you.
I Am Currently Being Cared For By A Consultant In The Hospital. Will The New Service Be Inferior To The Care I Am Getting Now?
No. If you have complex care needs the consultant will continue to support your care.
Does This Mean I Will Not Need To Go To The Hospital For My Diabetes At All?
Your clinical needs will continue to be assessed and if necessary you may be referred back to the hospital.
